CMS is requesting your pharmacy complete the HETS 3rd Party Attestation Form to verify your pharmacy’s relationship with eRx Network dba Change Healthcare as the submitter of patient eligibility (270/271) transactions related to your Medicare Part B DME and/or Medicare Part B Vaccine claims, if you have not completed this attestation already. This must be completed no later than 9/15/2025.
Please complete the following steps:
1. Begin the HETS Attestation Form by clicking the below link to access the HETS Electronic Data Interchange (EDI) Enrollment:

2. Select the link for your Medicare DME and/or Medicare Part B Vaccine jurisdiction.
3. Locate the HETS Attestation user guide for your Medicare DME and/or Medicare Part B Vaccine jurisdiction.
4. Follow the prompts and complete each required field. You will need the following information:
Unique ID = VUK5 (This will match eRx Network DBA Change Healthcare)
5. Continue to follow the prompts until the form has been fully submitted.
6. You should receive notification the request has been completed.
